Community Awards

Horizon Award

Special individuals and groups enrich our community by using their time, resources, and talents to make Lincoln a healthier place to live. CHE's Horizon Award recognizes the contributions of these individuals and groups.

The landscape of health care has shifted dramatically in recent years. In a time of uncertainty, a community of agencies came together to create Health 360, a model of collaboration and innovation on a state-of-the-art campus in the heart of Lincoln.

For the extraordinary collaboration that makes this possible in Lincoln, the agencies of the Health 360 Community - Lutheran Family Services, People's Health Center, Legal Aid of Nebraska, Community Crops, Community Action Partnership of Lancaster and Saunders Counties, Genoa Qol Pharmacy, The Bridge Behavioral Health, and the Lincoln-Lancaster County Health Department - collectively receive CHE's 2017 Horizon Award.
